Passport applications are accepted Wednesdays between 8:00 a.m. and 4:00 p.m. at the Recorder's Office located on the main floor of the Carroll County Courthouse.  Other days may be scheduled by appointment only. 712-792-1022 ext. 1

The processing time for a passport application is currently 10-13 weeks. See current processing times. For quicker processing, your application may be expedited for an additional $60 fee, collected at the time you submit your application.

For quicker processing, a $60 expedite fee will be collected at the time your application is submitted. Overnight mailing service is also available for an additional fee. Current expedited processing time is 7-9 weeks.

Passport Fees

Applications for new passports (Form DS-11) must be brought to the Environmental Health Office for processing. Passport applications may be obtained at our office or the National Passport Agency.

Items Required to Obtain a Passport:

1. Completed passport application (black ink only).
2. One passport photo (Photos are available for a $15 fee).
3. CERTIFIED Birth Certificate (Birth Certificate from the hospital does not meet standards) or ORIGINAL Citizenship Certificate.
4. For applicants 18 years of age or older:
   1. Must appear in person with a driver’s License or other acceptable photo ID
   2. Fee: $130 for passport or $30 for passport card – by check or money order only, payable to US Department of State
   3. Fee: $35 execution fee – by cash, check, or money order, payable to Carroll County
5. For applicants 16 or 17 years of age:
   1. Must appear in person with driver’s license and a check signed by a parent. If applicant does not have a driver’s license, one of the parents with a driver’s license must accompany the applicant.
   2. Fee: $130 for passport book or $30 for passport card – by check or money order only, payable to US Department of State
   3. Fee: $35 execution fee – by cash, check, or money order, payable to Carroll County
6. For applicants 15 years of age or younger:
   1. BOTH parents/legal guardians AND applicant must appear in person. In certain circumstances there are procedures that will allow a minor’s application to be accepted if one parent is unable to appear. Contact us for further information.
   2. Fees: $100 for passport or $15 for passport card – by check or money order, payable to US Department of State
   3. Fee: $35 execution fee – by cash, check, or money order, payable to Carroll County

Reminders:

• Complete your application with black ink.
• Allow plenty of time for your passport to be processed.
• You may expedite for $60.
• Do not sign your application until you are in the presence of a passport acceptance agent.
